          Hi Harold…

          20A: ‘abused’ is the definition. The rest of the clue is also a kind of definition – it’s saying that the answer sounds like an ad for a hospital. ‘Ill (people) treated (here)’

          5D: ‘theologian’ is DD because they may have a Doctor of Divinity and so write DD after their name. ‘Social welfare’ is the DOLE, so put DD into that. ‘Piece of cake’ (ie something very easy to do) is the definition.
